You (Requestor) agree that you are asking to voluntarily participate in a communication service (Service) provided by Miller Avenue Church of Mill Valley, California (MAC).
You understand and agree that the Prayer and Conversation Partners (PCPs) associated with MAC, its affiliates, and its related websites and other online, telephone, email, or other forms of communication, are available on a voluntary basis, are not professional therapists, counselors, or ordained ministers, and are not trained in any specific psycho-therapeutic methodology for interacting with you or other members of the public.
You further understand and agree that you are under no obligation to follow through with any part of the participation with a PCP after this initial request nor to take any advice, offered, implied, or inferred, from any PCP, nor to remain in contact or repeat contact with a PCP after the initial meeting via any method.
You further understand and agree that if you are suffering from any diagnosed mental illness or suspect such, that you will consult licensed and trained professional(s) in the psycho-therapeutic community in which you live. You agree, for yourself, your heirs, and your personal representatives, to defend, hold harmless, indemnify, release and forever discharge, to the broadest extent allowed by law, MAC, its trustees, officers, employees, agents, insurers, successors, and assigns from and against any and all claims, demands, actions, or causes of action on account of any real or perceived damage or suffering that may result from your participation in the Service.
